Once completed you can hand it in to any of the landlords listed on pages 19 and 20 of the form or post it to the:

EdIndex Team, Waverley Court C3, 4 East Market Street, Edinburgh, EH8 8BG.

The current cost to post the form using second class mail is £0.60.
